CAREER INFORMED CONSENT AGREEMENT AT VALENCIA HILLS COMMUNITY CHURCH * If you are interested in participating in the Career Coaching at Valencia Hills Community Church, please read and sign this Agreement. The Concept of Spiritual Care: The Career Coaching Ministry of Valencia Hills Community Church (the “Church”) provides biblically based spiritual and practical guidance that will seek to help you in your next steps career development. Outreach Ministry: The Church offers this ministry free of charge to both members and nonmembers of the Church. Not Professional Advice: Career Coaching is strictly a religious ministry outreach based on Scriptural principles, and is conducted under the authority and leadership of the Church. As a condition for receiving career coaching, you understand and agree that Career Coaches from the Church are not trained or licensed as mental health professionals, that career coaches are not designed to provide licensed professional mental health services. Termination of Spiritual Care: At any time, any party involved may terminate the ministry relationship. Confidentiality: Your Career Coach may disclose information to others for reasons including, but not necessarily limited to the following situations: 1. A reasonable determination by the Career Coach of abuse may require intervention to prevent harm. 2. Where a person threatens imminent harm to his or herself or another person (any safety issue), it may be necessary to intervene in order to prevent harm. Conflict Resolution: On rare occasions, a conflict may develop between a Career Coach and the and a person receiving support (coachee). If a conflict should arise between you and the Career Coach, and/or the Church, its officers and employees, you agree to resolve the dispute outside the secular court system. Any dispute will be settled by mediation with a mutually acceptable mediator. If necessary, the parties will move to legally binding arbitration, choosing an arbitrator mutually acceptable to both parties. Your signature below indicates your informed consent to the principles, policies and conditions of our Career Coaching Ministry that you agree to in order to participate in the Career Coaching Ministry. PLEASE CAREFULLY READ THE TERMS OF THIS CAREER COACHING INFORMED CONSENT AGREEMENT BEFORE SIGNING. If you have any questions about any of the terms, please talk with the Care Pastor.
